Summary

Lactate dehydrogenase (LDH) staining improves burn wound viability assessment consistency for both expert and novice clinicians. This method offers a more reliable evaluation than traditional hematoxylin and eosin (H&E) staining.

Area of Science:

  • Burn wound healing
  • Histopathology
  • Tissue viability assessment

Background:

  • Visual burn wound assessment often inaccurately predicts healing potential.
  • Hematoxylin and eosin (H&E) staining is subjective and difficult for novices.
  • Lactate dehydrogenase (LDH) staining presents a simpler, consistent alternative for burn tissue viability.

Purpose of the Study:

  • To evaluate the consistency and reliability of LDH staining for burn tissue viability.
  • To compare LDH staining with H&E staining in burn wound assessment.
  • To determine the utility of LDH staining for clinicians with varying expertise.

Main Methods:

  • 30 burn tissue samples from 6 patients were stained with H&E or LDH.
  • Histologic evaluation was performed by 3 raters with different skill levels.

  • Standardized checklist and scoring at 2 time points were used.
  • Main Results:

    • LDH staining demonstrated improved inter-rater agreement compared to H&E.
    • Agreement was highest for expert raters and lowest for novices with H&E.
    • LDH showed greater viability assessment agreement between experienced and novice clinicians.

    Conclusions:

    • LDH staining provides a more consistent method for assessing burn tissue viability.
    • LDH staining is a valuable tool for both experienced and novice clinicians.
    • This technique enhances the reliability of burn wound depth and healing potential evaluation.
