Background:

Ankle arthrodesis is standard for end-stage ankle arthritis but causes mobility loss.
Alternatives aim to preserve mobility and prevent adjacent joint arthritis.

Observation:

Four patients (32-42 years) with advanced ankle osteoarthritis underwent interpositional arthroplasty using acellular dermal matrix.
The procedure involved arthrotomy, debridement, microfracture, and talar resurfacing with the matrix, secured by anchors.

Findings:

Significant pain relief and improved tibiotalar joint range of motion (16.5° to 31°).
Mean Association of Orthopaedic Foot and Ankle Society scores improved from 35 to 88.5 at 12 months.
The procedure demonstrated success in enhancing function and motion while reducing pain.

Implications:

Acellular dermal matrix interpositional arthroplasty may be a viable alternative to ankle arthrodesis for young patients with end-stage ankle arthritis.
Further research with longer follow-up and histologic evaluation is recommended to confirm durability.
