Background:

  • Blepharospasm involves involuntary orbicularis muscle contractions causing eye twitching.
  • It can be primary or secondary to neurological issues, trauma, or medications.

Observation:

  • A 65-year-old woman experienced disabling episodes of eye closure for six months.
  • Neurological and systemic causes were excluded through thorough examinations and neuroimaging.

Findings:

  • Botulinum toxin (Botox) injection in one eye provided immediate relief to both eyes.
  • A subsequent placebo needle prick also resolved the eye closure, suggesting a psychogenic component.

Implications:

  • Differentiating functional disorders from true movement disorders is challenging.
  • Evaluating and managing patient stress is vital before considering invasive treatments for blepharospasm.
