Lenalidomide shows therapeutic effects in chronic lymphocytic leukemia (CLL). A specific gene expression profile, particularly high SHISA3 levels in leukemia cells, predicts patient response to lenalidomide treatment.
Area of Science:
- Oncology
- Immunology
- Genetics
Background:
- Lenalidomide is an effective therapeutic agent for chronic lymphocytic leukemia (CLL).
- Understanding predictive biomarkers for lenalidomide treatment is crucial for patient stratification.
Purpose of the Study:
- To identify gene expression profiles associated with lenalidomide response in CLL patients.
- To investigate the role of SHISA3 in lenalidomide sensitivity and Wnt signaling in CLL.
Main Methods:
- Phase II clinical trial involving 27 CLL patients treated with lenalidomide.
- Immune cell subset analysis (T lymphocytes, NK, monocytes, dendritic cells) at baseline and post-treatment.
- Gene expression analysis of pre-treatment leukemic cells from 16 CLL patients.
Main Results:
- No significant differences in immune cell levels or cytokines between responders (R) and non-responders (NR).
- CLL patients sensitive to lenalidomide exhibited a distinct gene expression profile.
- SHISA homolog 3 (SHISA3) was the most significantly upregulated gene in responders (fold change +23 vs. NR).
- SHISA3-high CLL cases showed restrained Wnt signaling activation and increased sensitivity to lenalidomide-induced apoptosis.
Conclusions:
- SHISA3 is a potential predictive biomarker for identifying CLL patients who will benefit from lenalidomide monotherapy.
- SHISA3 expression levels correlate with lenalidomide sensitivity and apoptosis induction in CLL.
