Background:

Mesothelioma of the tunica vaginalis testis (MTVT) is a rare neoplasm, typically affecting older males.
Asbestos exposure is a known risk factor for MTVT.
Scrotal enlargement is the primary clinical sign, with 15% of cases presenting with metastasis.

Observation:

A 62-year-old patient with MTVT, lacking asbestos exposure history, is presented.
Metastatic disease developed post-surgery, and chemotherapy led to rapid deterioration.
The case highlights challenges in early MTVT diagnosis due to non-specific symptoms and lack of tumor markers.

Findings:

Delayed diagnosis significantly correlates with an unfavorable prognosis in MTVT.
The efficacy of adjuvant chemotherapy and radiotherapy for MTVT remains uncertain.
MTVT has a generally poor prognosis, with a 30% mortality rate within 24 months.
