Blastic plasmacytoid dendritic cell neoplasm (BPDCN) is a rare cancer. Early diagnosis and treatment are crucial for improving patient outcomes in Mali, where this aggressive hematologic malignancy is often diagnosed late.

Background:
- Blastic plasmacytoid dendritic cell neoplasm (BPDCN) is a rare and aggressive hematologic malignancy.
- It typically presents with cutaneous lesions, but can also involve other organs.
Observation:
- A 19-year-old female in Mali presented with symptoms including anemia, fever, weakness, weight loss, and lymphadenopathy.
- Physical examination revealed pallor, fever, and enlarged inguinal and axillary lymph nodes.
- Ultrasonography showed hepatomegaly and splenomegaly.
Findings:
- Complete blood count indicated pancytopenia with severe anemia (Hemoglobin 4.8 g/dL), leukopenia (1900/μL), and thrombocytopenia (56,000/μL).
- Bone marrow biopsy and flow cytometry confirmed the diagnosis of BPDCN.
- The patient succumbed to septicemia four months post-admission due to delayed diagnosis.

- Timely diagnosis and access to targeted therapies, similar to those for acute leukemia, are essential for improving survival rates.
