Upgoing thumb sign: A sensitive indicator of brain involvement?

The upgoing thumb sign is more frequent in patients with minor stroke or transient ischemic attacks (TIAs) than in stroke mimics. This sign is a reliable indicator of brain involvement, showing good agreement among examiners and with patient symptoms.
