A case report details a kidney donor with a Bosniak 2F cyst who underwent partial cyst resection.
Pathology of the resected cyst revealed a multilocular cystic renal cell carcinoma, confirming malignancy.

Findings:

Partial resection of the Bosniak 2F cyst enabled successful kidney donation, benefiting both recipient and donor.
This intervention allowed the recipient to receive a kidney and the donor to avoid long-term surveillance.

Implications:

Presents a practical guideline for managing kidney donors with Bosniak 2F cysts, balancing transplant benefits against malignancy risks.
