Summary

This study introduces a new, minimally invasive lateral canthoplasty technique using gray line split and canthopexy for larger, brighter eyes. The procedure showed stable, satisfactory results with minimal complications in 87 patients.

Area of Science:

  • Ophthalmic plastic surgery
  • Aesthetic surgery

Background:

  • Asian women often desire larger, brighter eyes, with double eyelid surgery and epicanthoplasty being common procedures.
  • Aesthetic lateral canthoplasty, which lengthens the palpebral fissure, is increasingly popular but carries potential complications.
  • Existing lateral canthoplasty methods have limitations restricting their widespread use.

Purpose of the Study:

  • To present a novel, minimally invasive technique for aesthetic lateral canthoplasty.
  • To evaluate the efficacy and safety of the gray line split and canthopexy method for palpebral fissure lengthening.

Main Methods:

  • A minimally invasive approach involving gray line split and canthopexy was developed.
  • Eighty-seven patients underwent the procedure between February 2013 and January 2016.
  • Measurements of palpebral fissure length and photographic documentation were used for assessment, alongside complication monitoring.

Main Results:

  • Satisfactory aesthetic outcomes were achieved in nearly all 87 patients, with palpebral fissure lengthening ranging from 2.3 to 4.2 mm.
  • The observed lengthening of the palpebral fissure proved stable during long-term follow-up.
  • No severe complications such as lacrimal system fistula or hypertrophic scars were reported; one patient experienced conjunctival exposure.

Conclusions:

  • The gray line split method offers a safe and effective approach for achieving larger, brighter eyes through aesthetic lateral canthoplasty.
  • This technique demonstrates potential for wider adoption due to its favorable safety profile and satisfactory aesthetic results.
  • The method is considered a valuable advancement in aesthetic lateral canthoplasty.
