Summary

Coblation intracapsular total tonsillectomy (CITT) in children offers reduced pain and bleeding compared to conventional extracapsular tonsillectomy (CETT). This coblation tonsillectomy method also promotes faster wound healing with no observed regrowth after one year.

Background:

Adenotonsillar hyperplasia is a common condition in children requiring surgical intervention.
Tonsillectomy is a standard procedure for treating obstructive sleep apnea and recurrent tonsillitis.
Coblation technology offers an alternative to traditional tonsillectomy methods.

Purpose of the Study:

To compare the efficacy and complication rates of coblation intracapsular total tonsillectomy (CITT) versus conventional coblation extracapsular total tonsillectomy (CETT) in pediatric patients.
To evaluate intraoperative findings, postoperative recovery, and long-term outcomes including tonsil regrowth.

Main Methods:

A prospective study involving 90 children with adenotonsillar hyperplasia undergoing either CITT (48 patients) or CETT (42 patients).
Intraoperative bleeding and tonsillar fossa scores were recorded.
Patients were followed for at least one year to assess recovery, pain (Visual Analogue Scale), wound healing, and tonsil regrowth.

Main Results:

CITT group showed significantly lower intraoperative bleeding scores, tonsillar fossa scores, and postoperative pain (VAS) compared to the CETT group.
Faster healing and less inflammation were observed in the CITT group, indicated by the color of the white membrane postoperatively.
No instances of tonsil regrowth were detected in either group during the follow-up period.

Conclusions:

Coblation intracapsular total tonsillectomy (CITT) is associated with reduced intraoperative bleeding, less postoperative pain, and improved wound healing compared to conventional extracapsular tonsillectomy (CETT).
CITT presents a favorable option for pediatric tonsillectomy, enhancing patient recovery.
The study confirms the absence of tonsil regrowth after CITT, supporting its long-term efficacy.
