Inflammatory Bowel Disease II: Crohn's Disease01:30

Inflammatory Bowel Disease II: Crohn's Disease

1.4K
Introduction
Inflammatory bowel disease, commonly known as IBD, refers to a collection of disorders that lead to persistent inflammation of the gastrointestinal tract. The two types of IBD are ulcerative colitis, which impacts the colon, and Crohn's disease, which can involve any part of the gastrointestinal segment.
Crohn's disease
Crohn's disease is a chronic, systemic inflammatory bowel disease (IBD) that predominantly affects the gastrointestinal tract. It is marked by...

Inflammatory Bowel Disease V: Surgical Management

780
Surgical interventions for inflammatory bowel disease (IBD), which includes ulcerative colitis and Crohn's disease, are essential in managing symptoms and addressing complications. The selection of surgical procedures is contingent upon the specific conditions and complications that stem from these illnesses.
Here are some common surgical interventions for IBD:

Aneurysm III: Interprofessional Care

402
Aneurysm management involves either conservative medical therapy or surgical intervention, depending on the size and symptoms of the aneurysm. Conservative management is generally reserved for smaller, asymptomatic aneurysms, while larger or symptomatic aneurysms often necessitate surgical repair.Conservative Medical TherapyFor small, asymptomatic aneurysms, particularly abdominal aortic aneurysms (AAA) less than 5.5 centimeters in diameter, conservative medical therapy is recommended. This...

Chronic Bowel Disorders: Introduction

853
Chronic bowel diseases are a group of long-term conditions affecting the digestive tract, characterized by inflammation and damage to the gut lining. These conditions primarily include irritable bowel syndrome and inflammatory bowel disease.
Irritable Bowel Syndrome (IBS) is a common disorder affecting the gastrointestinal tract. The distinctive feature is recurrent abdominal pain associated with altered bowel movements, manifesting as constipation, diarrhea, or fluctuating between both. The...

Fungal Phylum Microsporidia

615
Microsporidia are a group of obligate intracellular fungi that were initially classified as protists but were later reclassified based on phylogenetic, molecular, and structural evidence linking them to the Chytridiomycota. These unicellular, non-motile organisms are highly specialized parasites that infect a wide range of animal hosts, including humans. They have evolved extensive genomic and metabolic reductions, making them highly dependent on their hosts for survival.Morphology and Genomic...

Skin Diseases and Disorders

5.9K
Skin is the first line of defense and encounters a variety of microbes. Some pathogenic strains are often the cause of a broad range of infections of the skin and other body systems. These conditions can affect people of all ages and may have different causes, including genetic factors, infections, autoimmune reactions, environmental factors, and lifestyle choices.
Gram-positive Staphylococcus spp. and Streptococcus spp. are responsible for many of the most common skin infections. However, many...

Summary

Actinomycosis, a rare but curable infection, should be considered for complex perianal fistulas. Prompt diagnosis and treatment with antibiotics led to significant improvement in a refractory case.

Area of Science:

  • Medical Microbiology
  • Gastroenterology
  • Surgical Infections

Background:

  • Perianal fistulizing disease often presents complex challenges in diagnosis and management.
  • Refractory cases necessitate consideration of uncommon etiologies beyond typical bacterial infections.

Observation:

  • A 26-year-old woman presented with complex perianal fistulae, including trans-sphincteric, suprasphincteric, and rectovaginal tracts.
  • Previous treatments involving abscess drainage, seton placement, and antibiotics yielded minimal improvement.
  • Anal cytology revealed the presence of Actinomyces species.

Findings:

  • Actinomycosis was identified as the underlying cause of the complex perianal fistulae.
  • A treatment regimen of intravenous penicillin followed by oral amoxicillin resulted in remarkable clinical improvement.

Implications:

  • This case highlights the importance of including actinomycosis in the differential diagnosis of refractory perianal fistulizing disease.
  • Early identification and targeted antibiotic therapy can lead to successful outcomes in challenging cases.
  • Considering rare infections is crucial for managing complex anorectal conditions.
