Circulating CD89-IgA complex does not predict deterioration of kidney function in Korean patients with IgA
Soluble CD89 (sCD89)-IgA complex does not predict disease progression in IgA nephropathy (IgAN). This study found no association between sCD89-IgA levels and adverse outcomes, limiting its clinical utility for IgAN risk stratification.
Area of Science:
- Nephrology
- Immunology
- Biomarker Discovery
Background:
- Soluble CD89 (sCD89)-IgA complex is implicated in IgA nephropathy (IgAN) pathogenesis.
- Evidence for sCD89-IgA as a biomarker for IgAN progression is limited.
- This study evaluated sCD89-IgA complex for IgAN risk stratification.
Purpose of the Study:
- To assess the utility of sCD89-IgA complex levels in predicting disease progression in IgA nephropathy (IgAN).
- To determine if sCD89-IgA complex can serve as a biomarker for risk stratification in IgAN patients.
Main Methods:
- Included 326 patients with biopsy-proven IgAN.
- Measured sCD89-IgA complex using sandwich-ELISA.
- Assessed 30% decline in estimated glomerular filtration rate (eGFR) as the primary endpoint.
Main Results:
- sCD89-IgA levels showed a weak inverse association with eGFR at biopsy (p=0.03), which lost significance in multivariate analysis (p=0.75).
- No association was found between sCD89-IgA complex levels and the risk of a 30% eGFR decline in Cox models.
- sCD89-IgA complex did not outperform clinical factors in predicting IgAN disease progression.
Conclusions:
- sCD89-IgA complex levels are not associated with disease progression in IgAN.
- Despite sCD89's role in immune complex formation, sCD89-IgA levels lack predictive value for adverse outcomes.
- The clinical utility of sCD89-IgA complex for risk stratification in IgAN is limited.
