Epilepsy is primarily characterized by unpredictable seizures, either provoked by an identifiable factor, such as injury or illness, or unprovoked, occurring spontaneously without apparent cause.

Summary

Status epilepticus (SE) in post-anoxic coma patients can have a better outcome than expected. Aggressive pharmacologic treatment may improve awakening rates in selected patients with favorable neurophysiological prognostication.

Background:

  • Status epilepticus (SE) carries a high mortality rate in post-anoxic coma.
  • The prognostic value of SE in this patient population requires further investigation.

Purpose of the Study:

  • To determine the awakening rate in post-anoxic comatose patients with SE.
  • To identify clinical, neurophysiological, and pharmacological factors correlating with outcomes.

Main Methods:

  • Retrospective analysis of seventeen post-anoxic patients with SE.
  • Univariate statistical analysis to correlate outcomes with various parameters.

Main Results:

  • Seven out of thirteen treated patients awakened, some with moderate-severe disability.
  • A significant correlation was found between awakening and preserved median nerve cortical somatosensory evoked potentials (SSEP), anesthetic use, and SE duration.

Conclusions:

  • SE in post-anoxic coma does not invariably predict a poor outcome.
  • Pharmacological treatment, including aggressive approaches, may be beneficial for patients with favorable multimodal prognostication patterns.
