Summary

Presacral neurectomy is the preferred surgical option for chronic pelvic pain and midline dysmenorrhea, especially with endometriosis. Combining it with uterine nerve ablation offers no additional benefit for pain relief.

Background:

Chronic pelvic pain and dysmenorrhea significantly impact reproductive-age women.
Medical management is often insufficient or contraindicated for severe cases.

Purpose of the Study:

To review surgical techniques and complications of pelvic denervation.
To evaluate evidence for pelvic denervation in primary and endometriosis-associated dysmenorrhea.

Main Methods:

Literature review of surgical pelvic denervation procedures.
Analysis of current evidence for efficacy and safety.

Main Results:

Presacral neurectomy is the preferred procedure for midline dysmenorrhea and endometriosis-related pain.
It serves as an adjunct to endometriosis lesion treatment for improved pain relief.
Combined presacral neurectomy and uterine nerve ablation shows no added patient benefit.
