Dysfunctional lens syndrome (DLS) occurs when internal eye optics, primarily the crystalline lens, cause poor visual quality. This condition can cause visual discomfort even without significant cataracts.

- Pre- and post-operative aberrometry revealed significant reductions in total higher-order aberrations (HOAs) and coma, particularly within the internal optics.
Findings:
- Pre-operative root-mean-square (RMS) of total higher-order aberrations was 0.350 μ, decreasing to 0.257 μ post-operatively.
- Pre-operative internal coma was 0.274 μ, significantly reduced to 0.131 μ post-operatively, indicating improved lens function.
Implications:
- HOAs originating from the crystalline lens can significantly impact visual quality when not adequately compensated by the cornea.
- These findings highlight the importance of evaluating internal lens optics in patients experiencing unexplained visual discomfort.
- Understanding DLS is crucial for accurate diagnosis and management of visual disturbances not attributable to refractive error or cataracts.
