A case study documented a patient who developed a chronic conjunctival granuloma.
This complication arose subsequent to undergoing pulley fixation suture placement on the medial rectus muscle.

Findings:

The study reports a rare instance of chronic conjunctival granuloma formation.
This adverse event occurred following a pulley fixation suture procedure on the medial rectus muscle.

Implications:

This case underscores a potential, albeit uncommon, complication associated with pulley fixation sutures.
Ophthalmologists should be aware of this risk and consider vigilant postoperative monitoring for granuloma development.
Further investigation may be warranted to understand the pathogenesis and prevention of such granulomas.
