Gonadal vein transposition offers an effective surgical solution for Nutcracker syndrome, a condition causing left renal vein compression. This approach provides symptom relief and avoids complications associated with other treatments.

  • Nutcracker syndrome involves mesoaortic compression of the left renal vein, leading to venous hypertension and symptoms like flank pain and hematuria.
  • Traditional treatments such as open left renal vein transposition have high reintervention rates (up to 30%) and associated surgical morbidity.
  • Endovascular stenting, while less invasive, has reported complications including stent fracture, thrombosis, and migration.

Observation:

  • A 26-year-old woman presented with intermittent flank pain and hematuria, diagnosed with Nutcracker syndrome via ultrasound and imaging.
  • Catheter venography confirmed left renal vein compression and revealed an 11 mm dilated gonadal vein contributing to pelvic varices.

Findings:

  • The patient underwent successful surgical transposition of the gonadal vein to the left common iliac vein.
