The Thyroid Gland01:23

The Thyroid Gland

8.0K
The thyroid gland is a small, butterfly-shaped gland located in the neck and covers the anterior surface of the trachea. The gland has two lateral lobes connected by a thin tissue mass called the isthmus. Internally, each lobe comprises many small spherical structures known as thyroid follicles, surrounded by a network of blood vessels.
The follicles have a central cavity lined by simple cuboidal to squamous epithelial cells called follicular cells. These cells produce the glycoprotein...

    Summary
    This summary is machine-generated.

Incidental thyroid carcinomas, often papillary and under 1 cm, are increasingly detected. Total thyroidectomy is recommended due to high rates of multifocality and occult lymph node metastasis.

    Area of Science:

    • Endocrinology
    • Oncology
    • Surgical Sciences

    Background:

    • Incidental thyroid carcinomas are small, often asymptomatic thyroid cancers discovered during evaluation for other conditions.
    • These tumors, typically papillary thyroid carcinoma, present diagnostic and therapeutic challenges.

    Purpose of the Study:

    • To analyze the incidence and characteristics of incidental thyroid carcinomas over 15 years.
    • To discuss therapeutic strategies and surgical management for these tumors.

    Main Methods:

    • Retrospective analysis of 83 patients with incidental thyroid carcinoma from 1793 thyroidectomies (2001-2015).
    • Evaluation included clinical, laboratory, and instrumental assessments, with Whole-Body Scan (WBS) being crucial for postoperative follow-up.

    Main Results:

    • Incidental thyroid carcinoma incidence was 4.62% (21.9% of all thyroid cancers).
    • Multifocality (15.66%) and bilaterality (7.23%) were common. All were papillary carcinomas, 3-10 mm in size.
    • Postoperative WBS detected lymph node metastasis in 57.69% of patients.

    Conclusions:

    • A significant increase in incidental thyroid carcinoma incidence was observed.
    • Total thyroidectomy is favored due to multifocality, bilaterality, and high rates of occult lymph node metastasis.
    • The findings support aggressive surgical management for optimal follow-up.
