Human acellular dermal matrices (HADM) and porcine acellular dermal matrices (PADM) show lower complication rates in breast reconstruction compared to bovine acellular dermal matrices (BADM). HADM and PADM are recommended for breast reconstruction due to better safety profiles.
Area of Science:
- Plastic Surgery
- Biomaterials Science
- Regenerative Medicine
Background:
- Acellular dermal matrices (ADMs) are widely used in breast reconstruction.
- ADMs are available from human (HADM), porcine (PADM), and bovine (BADM) sources.
- Long-term comparative studies are needed to evaluate complication rates among different ADM types.
Purpose of the Study:
- To compare complication rates and outcomes of HADM, PADM, and BADM in breast reconstruction.
- To evaluate the incidence of Red Breast Syndrome and capsular contracture with different ADMs.
Main Methods:
- Retrospective analysis of 52 ADM-breast reconstructions (oncologic and revision augmentation) from 2010-2015.
- Implantation of 15 HADMs, 21 PADMs, and 16 BADMs.
- Median follow-up of 36 months to assess complications.
Main Results:
- Overall complication rate was 17% (HADM: 7%, PADM: 14%, BADM: 31%).
- Composite endpoint of complications and Red Breast Syndrome showed lower event probability for HADM (7%) and PADM (19%) versus BADM (44%).
- Capsular contracture occurred in 6% of cases.
Conclusions:
- HADM and PADM are recommended for ADM-based breast reconstruction due to lower complication rates.
- BADM exhibited the highest complication probability but offered advantages in flexibility and aesthetics.
- Individual patient assessment is crucial for selecting the appropriate ADM for breast reconstruction.
