The Canadian Health Advanced by Nutrition and Graded Exercise (CHANGE) program integrates dietitians and kinesiologists into primary care to help patients with metabolic syndrome adopt healthier lifestyles. This interprofessional approach shows promise for overcoming barriers and improving patient health outcomes.
