Acute Pancreatitis II: Clinical Manifestations and Management01:30

Acute Pancreatitis II: Clinical Manifestations and Management

957
Acute pancreatitis presents a complex medical emergency characterized by rapid onset inflammation of the pancreas, demanding timely diagnosis and management to prevent complications. The condition primarily manifests through severe upper abdominal pain that often radiates to the back. This pain intensifies following the consumption of fatty foods. Accompanying symptoms such as nausea, vomiting, abdominal distention, fever, dyspnea, cyanosis, and jaundice can vary in intensity but significantly...

Effect of Hepatic Disease on Pharmacokinetics: Pathophysiologic Assessment and Liver Function Test

244
In clinical practice, the direct measurement of hepatic blood flow to evaluate liver function presents significant challenges due to the intricate and specialized nature of the necessary techniques. Consequently, healthcare professionals often rely on empirical estimates derived from thorough patient examinations and liver function tests to gauge liver health. Among the tools at their disposal, the Child–Pugh and MELD scoring systems stand out for their ability to categorize and assess...

Acute Kidney Injury III: Clinical Manifestations

1.1K
Acute Kidney Injury (AKI) progresses through distinct clinical phases: the oliguric, diuretic, and recovery phases, each marked by unique manifestations and challenges.Oliguric Phase:The oliguric phase is the initial stage of AKI, typically lasting 10 to 14 days. This phase is marked by a significant reduction in urine output, usually less than 400 mL per day, indicating decreased kidney function. Fluid retention is a prominent feature, leading to symptoms such as edema, hypertension, and...

Esophageal Varices-II: Clinical Features and Management

685
Esophageal varices often manifest as gastrointestinal bleeding episodes, presenting symptoms like hematemesis (vomiting of blood), hematochezia (passing fresh blood via the rectum), and melena (black, tarry stools). Other signs can include weight loss, anorexia, abdominal discomfort, jaundice, pruritus, altered mental status, and muscle cramps.
In the initial assessment, a thorough review of the patient's medical history is vital to identify risk factors such as liver disease, alcohol...

Effect of Hepatic Disease on Pharmacokinetics: Drug Dosing and Hepatic Blood Flow

298
Chronic liver disease significantly impacts drug metabolism due to alterations in hepatic blood flow and enzyme accessibility. This disruption affects the body's pharmacokinetics—the movement and processing of drugs within the system. Key enzymes crucial for metabolizing medications become less accessible, changing how drugs are processed and utilized. Furthermore, liver disease influences the synthesis of plasma proteins, such as albumin and globulins, which play critical roles in drug...

Summary

Multiple myeloma (MM) rarely causes liver failure. This case highlights MM presenting as acute liver injury, emphasizing its importance in differential diagnoses for liver decompensation.

Area of Science:

  • Hematology
  • Hepatology
  • Oncology

Background:

  • Multiple myeloma (MM) is a plasma cell malignancy that rarely presents with liver failure.
  • Acute liver injury and decompensation typically have diverse etiologies, often excluding hematologic malignancies.

Observation:

  • A 79-year-old male presented with fatigue, anorexia, and signs of hepatic decompensation.
  • Liver biopsy revealed diffuse plasma cell infiltration, confirming hepatic involvement of MM.
  • Bone marrow biopsy confirmed the diagnosis of multiple myeloma.

Findings:

  • The patient's liver failure was attributed to extensive plasma cell infiltration from MM.
  • Despite initiating chemotherapy, the patient experienced rapid decompensation and succumbed to respiratory failure.
  • Hepatic MM at initial presentation was associated with a poor prognosis.

Implications:

  • Multiple myeloma should be included in the differential diagnosis for patients presenting with acute decompensated liver disease.
  • Early detection and prompt treatment of hepatic MM are crucial for potentially improving patient outcomes.
  • This case underscores the importance of considering rare causes of liver failure in clinical practice.
