Background:

  • Pancreatic duct stones are a known cause of pancreatitis.
  • Acute pancreatitis associated with pancreatic ductal calculi of biliary origin is exceptionally rare.

Observation:

  • A 25-year-old female presented with acute pancreatitis.
  • Imaging revealed acute pancreatitis with a dilated pancreatic duct and intraductal calculi, without signs of chronic pancreatitis.

Findings:

  • Conservative management for the acute episode was successful.
  • Surgical extraction of pancreatic ductal calculi using lateral pancreaticojejunostomy (Partington-Rochelle procedure) was performed after the acute phase resolved.
  • The patient experienced a remarkable recovery and remained healthy at 4-month follow-up.

Implications:

  • Lateral pancreaticojejunostomy can lead to better outcomes in cases of acute pancreatitis associated with pancreatic duct stones.
  • Extraction of pancreatic ductal calculi after resolution of acute pancreatitis is a safe and effective procedure.
  • Further studies are warranted to evaluate the specific outcomes of this procedure for acute pancreatitis caused by ductal calculi.
