Nursing responsibilities before cardiac catheterization include:Assess for allergies and establish baseline health status. Before cardiac catheterization, assess the patient for allergies to contrast dye. Perform a comprehensive baseline assessment, including vital signs, heart and breath sounds, and a neurovascular assessment of the extremities, noting distal pulses, skin color, and temperature. Instruct the patient to fast for 8-12 hours before the procedure. Evaluate baseline laboratory...

Cardiac Catheterization I: Pre-Procedure Overview

1.8K
Cardiac catheterization is an invasive diagnostic technique used to identify and evaluate structural and functional diseases of the heart and major blood vessels. This technique diagnoses congenital heart disease, coronary artery disease, valvular heart disease, and coronary spasms and assesses ventricular function. It helps guide treatment decisions, including the need for revascularization procedures like percutaneous coronary intervention (PCI) or coronary artery bypass grafting (CABG) and...

Area of Science:

  • Interventional Cardiology
  • Electrophysiology
  • Medical Device Technology

Background:

  • Device implantation is a cornerstone of interventional cardiology, especially in electrophysiology.
  • This study shares experiences with device implantation techniques at Hayatabad Medical Complex, Peshawar.

Purpose of the Study:

  • To evaluate the safety and efficiency of the axillary vein approach for permanent pacemaker implantation.
  • To document procedural times and complication rates associated with this technique.

Main Methods:

  • A retrospective study from June 2011 to December 2015.
  • Devices implanted via axillary vein; subclavian vein used as an alternative when necessary, employing Seldinger technique.
  • Key metrics included fluoroscopy time (<10 minutes) and total procedure time (<45 minutes).

Main Results:

  • 800 permanent pacemakers (PPM) implanted: 450 single-chamber and 350 dual-chamber.
  • No major bleeding; minor ooze managed with pressure dressing.
  • Four pneumothorax cases (3 required intubation, 1 conservative); 8 cases of lead dislodgment.

Conclusions:

  • The axillary vein approach is a safe and time-efficient method for permanent pacemaker implantation.
  • The technique demonstrates a low complication rate, making it a viable option in electrophysiology.
