    Summary

    Area of Science:

    • Rheumatology
    • Immunology
    • Internal Medicine

    Background:

    • Systemic vasculitis can present with overlapping features, challenging classification into distinct syndromes.
    • Some patients with polyangiitis overlap syndrome were misdiagnosed with hypersensitivity or isolated cutaneous vasculitis.
    • Cutaneous manifestations are frequent in polyangiitis overlap syndrome.

    Purpose of the Study:

    • To describe the clinical features and treatment outcomes of patients with polyangiitis overlap syndrome.
    • To evaluate the efficacy of cyclophosphamide and corticosteroid therapy in inducing and maintaining remission.

    Main Methods:

    • Prospective study of ten patients with unclassifiable systemic vasculitis.
    • Treatment involved cyclophosphamide (2 mg/kg/day) and corticosteroids (initially daily, then alternate-day).

    Related Experiment Videos

  • Assessed remission rates and duration of remission after therapy discontinuation.
  • Main Results:

    • All ten patients achieved complete remission with the prescribed therapy.
    • Eight out of ten patients maintained remission after discontinuing cyclophosphamide.
    • Mean follow-up was 58.4 months; mean remission duration was 45.9 months.

    Conclusions:

    • Polyangiitis overlap syndrome is a distinct systemic vasculitis entity.
    • Cyclophosphamide and corticosteroids are effective in inducing remission for this syndrome.
    • Long-term remission is achievable, though relapses may occur upon therapy cessation.
