Summary

Primary lung sarcoma (PLS) is rare and aggressive. Stereotactic ablative radiotherapy (SABR) shows promise as a safe and effective treatment for inoperable PLS, achieving a complete response in one case.

Background:

  • Primary lung sarcoma (PLS) is an exceptionally rare and aggressive malignancy.
  • Current treatment options, including conventional radiotherapy, have yielded suboptimal outcomes.
  • Surgical resection is the preferred treatment but not always feasible.

Observation:

  • A case of an 82-year-old male patient with inoperable primary lung leiomyosarcoma (2.5 cm) in the right lower lobe was reported.
  • The patient had no regional nodal or distant metastasis.
  • Stereotactic ablative radiotherapy (SABR) using volumetric modulated arc therapy was administered.

Findings:

  • SABR delivered a dose of 56 Gy in four fractions.
  • Two months post-treatment, computed tomography revealed a complete tumor response.
  • The patient experienced only mild respiratory symptoms, with no evidence of disease for 2 years.

Implications:

  • This case study suggests SABR is a potentially safe and effective treatment modality for primary lung sarcoma.
  • SABR's precise targeting and high biologically effective dose may overcome limitations of conventional radiotherapy.
  • Further research is warranted to establish SABR's role in managing PLS.
