Background:

  • Articular cartilage defects present a significant clinical challenge.
  • Autologous chondrocyte implantation (ACI) has shown promise for durable outcomes, but long-term follow-up data is limited.
  • No previous studies have reported on ACI outcomes with over 20 years of follow-up.

Purpose of the Study:

  • To evaluate the long-term clinical outcomes of first-generation ACI in patients with disabling knee cartilage defects.
  • To assess the efficacy of ACI as an alternative to prosthetic arthroplasty.
  • To provide a minimum 20-year follow-up for ACI treatment outcomes.

Main Methods:

  • A case series (Level of evidence, 4) reviewed prospectively collected data from 23 patients (24 knees) undergoing ACI.
  • Patients had symptomatic, full-thickness articular cartilage lesions treated with ACI.
  • Clinical outcomes were assessed using validated scores (modified Cincinnati Knee Rating System, WOMAC, SF-36), visual analog scale for pain, and patient satisfaction surveys. Kaplan-Meier survival analysis was performed.

Main Results:

  • The 20-year survival rate for ACI was 63%.
  • Among knees with retained grafts, significant improvements in clinical scores (except WOMAC stiffness and SF-36 mental component) were sustained over 20 years.
  • Ninety-three percent of patients with retained grafts reported good or excellent knee-specific outcomes, and 79% maintained their native knee at 20 years.

Conclusions:

  • First-generation ACI offers satisfactory survival rates and sustained clinical benefits for knee cartilage defects over a 20-year period.
  • This study provides a crucial long-term benchmark for evaluating newer ACI technologies.
  • ACI can be a viable option for patients seeking to avoid prosthetic arthroplasty, with a majority maintaining their native knee function.
