Open Angle Glaucoma: Treatment

In open-angle glaucoma, the iridocorneal angle remains open, but the trabecular meshwork becomes stiff, slowing down the outflow of aqueous humor. Summary

Herpetic endophthalmitis can occur after cataract surgery. Prompt treatment with oral valacyclovir resolved inflammation when antibiotics failed.

Background:

Cataract surgery, specifically phacoemulsification, is a common procedure.
Postoperative endophthalmitis is a rare but serious complication following intraocular surgery.

Observation:

A 65-year-old male developed severe intraocular inflammation (herpetic endophthalmitis) in the 4th week after uneventful cataract surgery.
Initial treatments including intravitreal antibiotics and pars plana vitrectomy were ineffective.
Cultures for bacterial, mycobacterial, and fungal pathogens were negative.

Findings:

Pathological examination showed mononuclear and multinuclear giant cells, suggesting an inflammatory process.
Serological tests confirmed active herpes simplex virus (HSV) infection with positive IgM and IgG antibodies.
Diagnosis of herpetic endophthalmitis was established.

Implications:

Herpetic endophthalmitis should be considered in the differential diagnosis of postoperative inflammation after cataract surgery, even with negative cultures.
Early diagnosis and appropriate antiviral therapy (oral valacyclovir) are crucial for successful management.
This case highlights the importance of considering viral etiologies in refractory postoperative endophthalmitis.
