    Papillary thyroid carcinoma with lymph node metastases can be indolent. Long-term suppressive thyroid hormone therapy may be effective, suggesting less invasive treatments could be considered.

    Background:

    • Papillary thyroid carcinoma (PTC) is a common thyroid malignancy.
    • Cervical lymph node metastases are frequent in PTC.
    • Standard treatment often involves aggressive surgical and radioactive iodine approaches.

    Observation:

    • Two patients with PTC and cervical lymph node metastases were treated with suppressive doses of thyroid hormone.
    • These patients remained asymptomatic for 25 and 34 years post-treatment.
    • The local metastases appeared to be relatively innocuous.

    Findings:

    • Long-term suppressive thyroid hormone therapy was associated with prolonged asymptomatic disease in patients with PTC lymph node metastases.
    • Evidence suggests these metastases may have a protective effect in this low-grade cancer.
    • The observed outcomes challenge the necessity of aggressive treatment modalities.

    Implications:

    • Current treatment paradigms for PTC with lymph node metastases may be overly invasive.
    • Reassessment of treatment strategies, potentially favoring less invasive options like hormone suppression, is warranted.
    • Further research into the indolent nature of certain PTC metastases is needed.
