Summary

Acid etching teeth for 15 seconds with phosphoric acid does not increase demineralization risk during orthodontic treatment. This finding suggests that standard acid etching protocols do not heighten the likelihood of white spot lesions in orthodontics.

Background:

  • The clinical significance of acid etching before orthodontic bonding remains debated.
  • Enamel demineralization is a concern during orthodontic treatment.

Purpose of the Study:

  • To evaluate the effect of 15-second acid etching on enamel demineralization.
  • To determine if initial demineralization increases susceptibility to further demineralization.

Main Methods:

  • Human molars were analyzed using FluoreCam imaging before and after etching.
  • One group received 15-second acid etching; the control group received only a water rinse.
  • Calcium leaching was measured using inductively-coupled plasma auger electron spectrometry.
  • Both groups underwent 9 days of pH cycling to simulate oral conditions.

Main Results:

  • Acid etching significantly increased enamel lesion area and decreased light intensity.
  • 15 seconds of etching released 14 ppm ±2.4 of calcium.
  • No significant difference in overall enamel demineralization was observed between groups after pH cycling.

Conclusions:

  • Initial demineralization from brief acid etching does not enhance enamel susceptibility to subsequent demineralization.
  • Acid etching for 15 seconds is unlikely to increase the risk of white spot lesions during orthodontic treatment.
