Background:

Aggressive angiomyxoma (AAM) is a rare mesenchymal tumor.
AAM predominantly affects women in the genital, perineal, and pelvic regions.

Observation:

This report describes two male patients with paratesticular AAM presenting as inguinal swellings.
Gross examination revealed globular masses with a greyish-white, glistening cut surface.

Findings:

Microscopic examination showed a paucicellular tumor with spindle cells, variable caliber vessels, and a myxocollagenous stroma.
One case exhibited fat infiltration.
Immunohistochemistry revealed CD34, desmin, and vimentin positivity, with negative staining for S100, actin, Estrogen Receptors (ER), and Progesterone Receptors (PR).

Implications:

This case series highlights the extreme rarity of paratesticular aggressive angiomyxoma in males.
The findings underscore the importance of considering AAM in the differential diagnosis of inguinal masses in men.
Understanding the infiltrative nature of AAM is crucial for surgical management and patient outcomes.
