  • Waldenstrom macroglobulinaemia (WM) is a rare lymphoplasmacytic lymphoma.
  • Central nervous system (CNS) involvement, known as Bing-Neel syndrome (BNS), is an uncommon complication of WM.

Observation:

  • A 68-year-old male presented with vision loss in his right eye.
  • The vision loss was caused by compressive optic neuropathy due to WM relapse in bilateral cavernous sinuses.

Findings:

  • This case represents the first reported instance of BNS simultaneously affecting bilateral cavernous sinuses.
  • The patient exhibited a significant clinical and radiological response to chemotherapy.

Implications:

  • Increased awareness of rare CNS manifestations like BNS is crucial as WM treatment outcomes improve.
  • Neurosurgical intervention may be necessary for diagnosis or decompression in such cases.
