Background:

Mediastinal masses are frequently diagnosed, presenting diverse differential diagnoses.
Mass location is crucial for narrowing diagnostic possibilities.
Common superior mediastinal masses include thyroid, thymus, germ cell tumors, and lymph nodes.

Observation:

Posterior mediastinal masses are typically neurogenic tumors, with schwannoma being the most frequent.
This report details a unique case of a cystic schwannoma.
The cystic schwannoma presented as a superior mediastinal mass.

Findings:

Schwannomas, typically posterior mediastinal, can rarely manifest in the superior mediastinum.
Cystic variants of schwannoma represent an uncommon presentation.
Location-based diagnostic criteria for mediastinal masses were challenged by this case.

Implications:

This case expands the differential diagnosis for superior mediastinal masses.
It underscores the importance of considering rare presentations of common tumors.
Radiological and pathological correlation is vital for accurate diagnosis of mediastinal lesions.
