Fractures: Bone Repair01:27

Fractures: Bone Repair

5.8K
Treatment for a fracture is based on the type of break, the bone affected, and the patient's age.
Minor fractures with no bone displacement are treated by immobilizing the fractured bone using a cast or splint. However, in the case of fractures with displaced bones, the broken bones are repositioned before immobilization to ensure successful healing without deformation and loss of function. The realignment of fractured bone ends is performed through a process called reduction. If the...

Summary

Closed reduction and percutaneous screw fixation for talar neck fractures show good outcomes. This minimally invasive technique for talus fractures reduces complications and promotes healing.

Area of Science:

  • Orthopaedic Surgery
  • Traumatology
  • Skeletal Repair

Background:

  • Talar neck fractures are uncommon, with displaced fractures posing a risk of avascular necrosis due to tenuous blood supply.
  • Minimally invasive techniques aim to reduce soft tissue damage and preserve blood supply to the talus.

Purpose of the Study:

  • To evaluate the outcomes and complications of closed reduction and percutaneous screw fixation for talar neck fractures.
  • To assess the efficacy of this minimally invasive approach in managing talus injuries.

Main Methods:

  • A cohort of 21 patients with talar neck fractures underwent closed reduction and percutaneous screw fixation between 2009 and 2014.
  • Preoperative 3D CT scans guided the procedure, with Kirscher wires for reduction and cannulated screws for fixation.
  • Patient outcomes were assessed using the American Orthopaedic Foot and Ankle Society (AOFAS) scoring scale.

Main Results:

  • Average follow-up was 32 months, with an average healing time of 15.5 weeks.
  • The mean AOFAS score was 82.3, with excellent results in 75% of type I and 72% of type II Hawkins classifications.
  • Complications included partial avascular necrosis in two patients and subtalar arthritis in three; no wound complications occurred.

Conclusions:

  • Preoperative 3D CT, early surgical intervention, and anatomic reduction with percutaneous screw fixation enhance talar fracture healing.
  • This technique effectively reduces the incidence of avascular necrosis and soft tissue complications in talar neck fractures.
