Background:

  • Daily cone-beam CT (CBCT) scans are standard for non-small cell lung cancer (NSCLC) patient positioning.
  • Radiomics offers quantitative insights for potential early treatment adaptation.

Purpose of the Study:

  • To develop and validate a feature selection methodology for longitudinal radiomics.
  • To identify optimal time-points during treatment for early response assessment.

Main Methods:

  • Analyzed CBCT scans from 90 NSCLC patients, including test-retest and weekly scans.
  • Extracted 116 radiomic features from the GTV, calculating delta features (absolute and relative differences).
  • Utilized smallest detectable change (C) from test-retest data for feature selection criteria.

Main Results:

  • The number of significant delta features increased from 6 at week 2 to 85 by week 5.
  • A 93% overlap was observed in features selected from week 3 onwards.
  • The methodology identified reproducible, informative delta radiomic features.

Conclusions:

  • A robust feature selection methodology for longitudinal radiomics was established.
  • Selected delta radiomic features show potential for informing adaptive radiotherapy decisions.
  • Further research is needed to confirm the prognostic value of these features.
