Background:

Purpose of the Study:

  • To evaluate the association between dynapenia and care complexity in male residents of Veterans Care Homes (VCHs).
  • To identify specific indicators of increased care needs in this population.

Main Methods:

  • Cross-sectional study of 504 male residents in Taiwanese VCHs.
  • Dynapenia defined by low handgrip strength (<26 kg).
  • Resident Assessment Protocol (RAP) triggers used to assess care complexity, with adjustments for age, education, BMI, and Charlson's Comorbidity Index (CCI).

Main Results:

  • Dynapenia and handgrip strength were not independently associated with higher numbers of RAP triggers after adjustments.
  • Slow gait speed (≤0.803 m/s) was significantly associated with higher care complexity (≥4 RAP triggers).
  • Slow gait speed correlated with specific RAP triggers including cognitive loss, communication deficits, rehabilitation needs, incontinence, falls, and psychotropic drug use.

Conclusions:

  • Slow gait speed is a more reliable indicator of higher care complexity than dynapenia in older male LTCF residents.
  • Gait speed assessment can help identify residents requiring intensified care management.
  • Findings suggest a need to incorporate gait speed evaluation into routine assessments for older adults in long-term care settings.
