Summary

Sciatic neuromuscular variants and piriformis muscle asymmetry are common findings on lumbosacral plexus MR neurography. Interobserver agreement for identifying these variants was excellent, while agreement for piriformis muscle asymmetry was moderate to good.

Background:

Sciatic nerve and piriformis muscle variations can impact clinical presentation.
Accurate identification of these anatomical variants is crucial for diagnosis.
MR neurography (MRN) is a key imaging modality for evaluating the lumbosacral plexus.

Purpose of the Study:

To determine the frequency of sciatic neuromuscular variants on MR neurography.
To assess the interobserver variability in identifying these variants and piriformis muscle asymmetry.

Main Methods:

Retrospective review of 137 lumbosacral plexus MRN examinations.
Nerve-selective 3D imaging was utilized.
Two independent readers evaluated for sciatic neuromuscular variants and piriformis muscle asymmetry, assessing inter- and intraobserver performance.

Main Results:

Sciatic neuromuscular variants were present in 16.4% of extremities.
Interobserver agreement for sciatic nerve variants was excellent (kappa 0.8-0.9).
Piriformis muscle asymmetry was found in 33.6% of patients, with moderate to good interobserver agreement (kappa 0.39-0.61).

Conclusions:

Sciatic neuromuscular variants and piriformis muscle asymmetry are frequent findings on lumbosacral plexus MRN.
High interobserver reliability supports the use of MRN for diagnosing these common variants.
These findings are consistent with cumulative prevalence data from existing literature.
