A rare congenital heart defect, congenital left ventricular diverticulum (CLVD), can cause sudden cardiac arrest. Surgical repair and defibrillator implantation successfully treated a young man with CLVD and ventricular fibrillation.

Observation:
- A previously healthy 20-year-old male presented with sudden ventricular fibrillation (VF).
- Intractable VF required cardiopulmonary resuscitation and venoarterial extracorporeal membrane oxygenation (ECMO) for circulatory support.
Findings:
- Cardiac MRI confirmed CLVD in the left ventricle's posterolateral wall.
- The patient had a history of sudden cardiac arrest due to VF.
Implications:
- Surgical repair of CLVD is crucial to prevent recurrent VF and left ventricular rupture.
- Implantable cardioverter-defibrillator (ICD) implantation provides long-term protection against life-threatening arrhythmias.
