Observation:

• A clinical observation of a 63-year-old male patient with bladder cancer (pT2N0M0G3) is presented.
• The patient underwent laparoscopic cystoprostatvesiculectomy with pelvic lymph node dissection and orthotopic ileal bladder substitution.
• The upper urinary tract was managed with ureteral stents and external diversion.

Findings:

• The Karolinska Institute technique, utilizing eccentric suturing, successfully created an oval-shaped neobladder.
• The resulting neobladder demonstrated anatomical and functional comparability to a healthy bladder.

Implications:

• This technique provides a viable surgical option for orthotopic bladder substitution.
• It may enhance functional outcomes and patient quality of life after radical cystectomy for bladder cancer.
