Summary
A Pakistani woman developed severe sepsis and acute respiratory distress syndrome after a miscarriage. Tuberculosis was diagnosed via peritoneal fluid analysis, leading to a full recovery with antituberculous treatment.

Background:
- A 23-year-old woman experienced a spontaneous miscarriage in the second trimester while in Saudi Arabia.
- Following the miscarriage, a dilatation and curettage (D&C) procedure was performed.
Observation:
- The patient subsequently developed sepsis and acute respiratory distress syndrome (ARDS), necessitating intensive care unit admission.
- Exploratory laparotomy revealed a pelvic abscess, and despite broad-spectrum antibiotics, she had persistent fever and respiratory compromise.
Findings:
- Peritoneal fluid analysis was positive for Tuberculosis PCR.
- Histopathology of an omental specimen showed necrotising granuloma, and ascitic fluid culture confirmed *Mycobacterium tuberculosis*.
Implications:
- This case highlights a rare presentation of disseminated tuberculosis following a gynecological procedure.
- Prompt diagnosis and initiation of antituberculous treatment led to a successful clinical recovery.
