Background:

Graft infection with secondary aortic fistula is a rare complication of aortic prosthesis implantation.
This condition significantly increases patient morbidity and mortality, often occurring after emergency surgeries or reoperations.

Observation:

A case of recurrent aortoenteric fistula in an elderly patient following surgery for a ruptured abdominal aortic aneurysm is presented.
The patient underwent aortic stentgraft implantation and fistula revision, but the original aortic prosthesis was retained.

Findings:

Despite two recurrences, the patient achieved long-term survival exceeding 12 years with a good quality of life.
This highlights the potential efficacy of non-curative management strategies in select high-risk cases.

Implications:

Non-curative treatment, including aortic stentgraft placement and long-term antibiotic therapy, can be a viable alternative when graft explantation is too risky.
This approach may offer a survival benefit and improved quality of life for patients with complex aortic graft infections and fistulas.
