    Summary

    Tamoxifen did not induce ovulation in clomiphene-resistant patients. However, it successfully induced ovulation in 84% of cycles for previously untreated patients with hypothalamic anovulation.

    Background:

    • Hypothalamic anovulation is a common cause of infertility.
    • Clomiphene citrate is a first-line treatment, but some patients are resistant.
    • Alternative ovulation induction agents are needed.

    Purpose of the Study:

    • To evaluate tamoxifen's efficacy in inducing ovulation in clomiphene-resistant patients.
    • To assess tamoxifen's effectiveness in previously untreated patients with hypothalamic anovulation.

    Main Methods:

    • Seventeen clomiphene-resistant patients received tamoxifen (10 mg/day, cycle days 5-9) for two cycles.
    • Forty-five treatment-naive patients with hypothalamic anovulation received tamoxifen (10 mg/day, cycle days 5-9).
    • Ovulation was monitored via ultrasound and serum estradiol levels.

    Main Results:

    • Tamoxifen failed to induce ovulation in 15 of 17 clomiphene-resistant patients.
    • One of the two responsive clomiphene-resistant patients conceived after hCG administration.
    • Tamoxifen induced ovulation in 84% of cycles in treatment-naive patients.
    • Improved cervical mucus was observed with tamoxifen compared to clomiphene.

    Conclusions:

    • Tamoxifen is not effective for clomiphene-resistant hypothalamic anovulation.
    • Tamoxifen shows promise for inducing ovulation in treatment-naive patients with hypothalamic anovulation.
    • Tamoxifen may offer advantages over clomiphene in certain patient populations.
