Background:

  • Inflammatory pseudotumors (IPTs) are rare, benign neoplastic-like lesions composed of inflammatory cells and stromal proliferation.
  • IPTs can occur in various locations, but mesosalpingeal (fallopian tube) involvement is exceptionally uncommon.
  • Distinguishing IPTs from malignant neoplasms and other benign conditions is crucial for appropriate patient management.

Observation:

  • A case of a mesosalpingeal mass with distinct clinical and pathologic features is presented.
  • The mass exhibited characteristics suggestive of an inflammatory process rather than a typical neoplasm.
  • Diagnostic imaging and histopathological examination were essential in characterizing the lesion.

Findings:

  • The final diagnosis was inflammatory pseudotumor based on integrated clinicopathologic findings.
  • The differential diagnosis considered included Castleman disease and immunoglobulin G4-related fibrosclerosing tumor.
  • Histologic examination revealed a mixed inflammatory infiltrate and fibrosis, consistent with IPT.

Implications:

  • This case expands the known spectrum of locations for inflammatory pseudotumors.
  • Accurate diagnosis of mesosalpingeal IPT is vital to avoid misclassification as malignancy, ensuring appropriate treatment.
  • Recognition of this entity aids in managing rare pelvic masses and understanding their diverse origins.
