Long-term tiotropium use improved lung function in patients with mild to moderate chronic obstructive pulmonary disease (COPD). It did not significantly alter the annual decline in FEV1 before bronchodilator use but did reduce the decline after bronchodilator use.

- A multicenter, randomized, double-blind, placebo-controlled trial involving 841 patients with Global Initiative for Chronic Obstructive Lung Disease (GOLD) stage 1 or 2 COPD.
- Patients received daily inhaled tiotropium (18 μg) or placebo for 24 months.
- Primary endpoint: change in pre-bronchodilator FEV1 at 24 months. Secondary endpoints: post-bronchodilator FEV1 changes and annual FEV1 decline (pre- and post-bronchodilator).

- Tiotropium consistently resulted in higher FEV1 compared to placebo throughout the trial (P<0.001).
- No significant difference in the annual decline of pre-bronchodilator FEV1 between groups (P=0.06).
- A significant reduction in the annual decline of post-bronchodilator FEV1 was observed with tiotropium (22 ml/year, P=0.006).
- Adverse event incidence was similar between groups.
Conclusions:
- Tiotropium improved FEV1 at 24 months in patients with mild to moderate COPD.
- Tiotropium use ameliorated the annual decline in post-bronchodilator FEV1 in this population.
- The findings support the use of tiotropium for managing lung function in early-stage COPD.
