Summary

This study explored using Endpoint Management (EM) software to enable subthreshold laser therapy with conventional retinal lasers. Findings suggest EM algorithm approximations may allow for effective subthreshold treatment, making advanced therapies more accessible.

Purpose of the Study:

  • To investigate the feasibility of using the Endpoint Management (EM) algorithm from advanced devices for subthreshold laser treatment.
  • To assess if EM software can enable subthreshold retinal laser therapy using conventional systems.
  • To evaluate the efficacy of EM-based subthreshold laser treatment in a rabbit model.

Main Methods:

  • A rabbit model (eight eyes from four Dutch-Belted rabbits) was used.
  • Two devices, PASCAL Streamline 577 and MC 500-Vixi™, were employed.
  • Laser methods included EM, PASCAL Streamline 577-Subthreshold (PSS-SDM), and MC500-VixiTM (VX-SDM) with varying energy settings.
  • Optical coherence tomography (OCT) and histological findings were used to assess tissue damage.

Main Results:

  • The spot size achieved with EM at 50% setting was 7183 μm2.
  • PSS-SDM and VX-SDM required 50-60 mW to achieve comparable spot sizes.
  • Tissue damage at 50 mW for PSS-SDM and VX-SDM corresponded with the outer and inner nuclear layers observed via OCT and histology.

Conclusions:

  • The study suggests that the EM algorithm can be approximated for subthreshold laser therapy.
  • This approach may enable the use of subthreshold laser treatment with conventional retinal laser therapy systems.
  • Findings indicate a potential pathway for wider accessibility of advanced laser therapies.
