Summary

Abdominal radical trachelectomy (ART) often alters menstrual patterns, with cervical stenosis being a common cause. Tailed intrauterine devices (IUDs) effectively prevent stenosis, and cervical dilation treats existing issues, improving menstrual health post-surgery.

Area of Science:

  • Gynecologic Oncology
  • Reproductive Medicine
  • Surgical Outcomes

Background:

  • Abdominal radical trachelectomy (ART) is a fertility-sparing procedure for early-stage cervical cancer.
  • Post-surgical menstrual pattern changes are a significant concern for patients undergoing ART.
  • Understanding these changes is crucial for patient counseling and management.

Purpose of the Study:

  • To evaluate menstrual pattern alterations following ART.
  • To identify the causes of these menstrual changes.
  • To assess the effectiveness of interventions like cervical dilation and intrauterine devices (IUDs) for prevention and treatment.

Main Methods:

  • A retrospective study of 129 patients who underwent ART.
  • Categorization of patients into three groups based on post-operative menstrual status (unchanged, changed without amenorrhea, amenorrhea).

  • Analysis of causes, including cervical stenosis, and evaluation of IUD placement and cervical dilation outcomes.
  • Main Results:

    • The majority of patients (87.6%) experienced changes in menstrual patterns post-ART.
    • Cervical stenosis, due to narrowing of the cervical os, was the primary cause of altered menstruation.
    • Tailed IUDs were associated with a lower incidence of cervical stenosis, and cervical dilation improved menstrual conditions in most cases.

    Conclusions:

    • Menstrual pattern changes are common after ART, primarily caused by cervical canal narrowing.
    • Cervical dilation is an effective treatment for post-ART cervical stenosis.
    • Tailed IUDs demonstrate efficacy in preventing cervical stenosis, thereby preserving regular menstruation.
