Purpose of the Study:

  • To systematically review and meta-analyze the diagnostic yield of image-guided biopsy for infectious discitis.

Main Methods:

  • Searched four major databases (PubMed, Cochrane, Embase, Scopus) up to March 2016.
  • Included 33 peer-reviewed articles on patients with suspected discitis undergoing image-guided biopsy.
  • Pooled data to estimate yield using inverse variance method and logit transformation.

Main Results:

  • Image-guided biopsy demonstrated a diagnostic yield of approximately 48% (793/1763).
  • This yield is significantly lower than open surgical biopsy (76%; 152/201; P < .01).
  • Prior antibiotic exposure did not significantly alter biopsy yield (32% vs. 43%; P = .08).

Conclusions:

  • Image-guided biopsy has a moderate diagnostic yield for infectious discitis.
  • Open surgical biopsy offers a significantly higher yield.
  • Prior antibiotic use does not significantly affect the diagnostic yield of image-guided biopsy.
