Summary

Periocular radiotherapy significantly impacts meibomian glands, leading to dysfunction and dry eye symptoms. Even low radiation doses can cause significant meibomian gland dropout and tear film instability.

Background:

  • Meibomian gland dysfunction (MGD) is a common cause of evaporative dry eye.
  • Periocular radiotherapy is used to treat ocular and orbital malignancies.

Purpose of the Study:

  • To investigate the effects of periocular radiotherapy on meibomian gland function and ocular surface parameters.

Main Methods:

  • A comparative study involving 28 patients (40 eyes) who received radiotherapy and 30 age-matched controls (60 eyes).
  • Evaluated parameters included slit-lamp examination, Schirmer test, meibography, tear film breakup time (TBUT), Ocular Surface Disease Index (OSDI) scores, meibomian gland evaluation (meiboscore, meibum expressibility, lid margin abnormality), and tear film lipid layer thickness.

Main Results:

  • The radiotherapy group showed significantly higher meiboscores, meibum expressibility, and OSDI scores compared to controls.
  • The radiotherapy group exhibited lower Schirmer values, TBUT, and lipid layer thickness.
  • Meibomian gland dropout correlated with age and total radiation dose, with even low doses (<30 Gy) causing significant MGD and reduced TBUT.

Conclusions:

  • Periocular radiotherapy induces meibomian gland dysfunction, resulting in tear film instability and severe dry eye symptoms.
  • These findings highlight the ocular surface toxicity of radiotherapy and the need for management strategies for radiation-induced dry eye.
