Background:

  • Orthostatic hypotension (OH) is a sustained blood pressure drop upon standing.
  • Delayed OH occurs after three minutes of upright posture.
  • OH can lead to significant morbidity if not identified.

Purpose of the Study:

  • To determine the incidence of delayed orthostatic hypotension (delayed OH) in patients referred to an autonomic lab.
  • To investigate associated symptoms and other autonomic function test abnormalities in patients with delayed OH.

Main Methods:

  • Noninvasive blood pressure monitoring for 45 minutes during tilt table testing.
  • Inclusion of patients over 18 years referred for autonomic function tests.
  • Exclusion of patients with severe cardiac issues or neurally mediated syncope.

Main Results:

  • Fifty out of 170 patients exhibited delayed OH (blood pressure fall after 3 minutes).
  • Symptomatic delayed OH occurred in 29 patients; asymptomatic cases were more frequent after 10 minutes.
  • This pilot study is the first in India to report on delayed OH incidence.

Conclusions:

  • Delayed orthostatic hypotension is a significant finding that may be overlooked in routine clinical assessments.
  • High clinical suspicion and tilt table testing are recommended for diagnosing delayed OH.
  • Early detection and management of delayed OH are essential to mitigate patient morbidity.
