    Background:

    • Improvised explosive devices and landmines frequently cause pelvic fractures, leading to life-threatening hemorrhage.
    • Pelvic fractures are a significant concern in modern warfare, necessitating effective hemorrhage control strategies.

    Purpose of the Study:

    • To compare the efficacy of an improvised pelvic binder (combat trouser binder - CTB) with a commercially available trauma pelvic orthotic device (TPOD).
    • To evaluate intrapelvic pressure changes induced by both pelvic binding methods in a cadaveric model.

    Main Methods:

    • Six unembalmed cadavers underwent simulated unstable pelvic fractures (AO/OTA 61-C1).
    • Intrapelvic pressure was measured using a manometric balloon in the retropubic space, with a baseline pressure of 8cm H2O.
    • The CTB and TPOD were applied sequentially, and steady intrapelvic pressures were recorded and statistically analyzed.

    Main Results:

    • Both the CTB and TPOD significantly increased intrapelvic pressure compared to baseline (p < .01 and .036, respectively).
    • The median steady intrapelvic pressure was 16cm H2O for the CTB and 18cm H2O for the TPOD.
    • There was no statistically significant difference in intrapelvic pressure between the CTB and TPOD (p > .05).

    Conclusions:

    • The combat trouser binder (CTB) is a novel, rapidly deployable, and effective method for pelvic binding.
    • This improvised technique can tamponade and control intrapelvic hemorrhage in austere environments.
    • The CTB offers a practical solution for managing pelvic fractures in combat casualties.
