Summary

Complete excision with a narrow surgical margin may be effective for treating cutaneous leiomyosarcoma. This case study shows successful management of facial cutaneous leiomyosarcoma with a 1 cm margin and no recurrence after 1.5 years.

Purpose of the Study:

  • To present a case of cutaneous leiomyosarcoma managed successfully with a narrow surgical margin.
  • To evaluate the efficacy of narrow margin excision for facial cutaneous leiomyosarcoma.

  • A 74-year-old woman with a rapidly growing facial mass underwent biopsy, confirming cutaneous leiomyosarcoma.
  • Complete surgical excision with a 1 cm lateral margin was performed.
  • The defect was repaired using bilateral V-Y advancement local flaps. Histopathology and immunohistochemistry confirmed the diagnosis and negative margins.

Main Results:

  • Histopathology confirmed a moderately differentiated cutaneous leiomyosarcoma with negative margin involvement.
  • The patient showed no locoregional recurrence during a 1.5-year postoperative follow-up.
  • No additional treatment was administered or required.

Conclusions:

  • Cutaneous leiomyosarcomas can be successfully treated with complete excision and a narrow surgical margin.
  • Regular postoperative follow-up is crucial for monitoring.
  • This approach offers a viable alternative for managing facial cutaneous leiomyosarcomas.
